Insurance Phaseout and Payment Information

Most Insurance plans accepted until July 2025



Dr. Sullivan and her physician assistants accept most insurance plans. They are preferred/ in-network providers for those listed below. Please note that your insurance website may not reflect recent contract changes. You should check with your insurance plan to verify your specific benefits. As a courtesy, we also check your benefits and will relay this information to you before your initial visit.

Allergists are highly specialized and use a number of individual testing reagents to help with diagnosis and management.

Insurance reimbursement is per reagent so the total cost for a visit may vary.

You may call your insurance company for an estimation ahead of time using the insurance procedure codes below.

You may also use diagnostic code J30.1 (allergic rhinitis) to check your benefits, even if this code does not apply. Your practitioner will submit the claim with a code that is appropriate for you at the time of service.


New Patient Procedure Codes and approximate insurance discounted cost:


99205- Initial visit- Comprehensive 60 minutes $190-$380


99417- Extra time by provider for counseling $40-120 per 15 min.


94375 or 94060- Simple or Pre/Post Spirometry for ages 6 and up $40-$80


95004 Prick skin tests (environment and food) x 48 units + select foods for ages 6 yo- adult $250-$450

Infant- Age 5 yrs

Established Patient Procedure Codes and approximate insurance discounted cost:


99211-99215- Time based routine visit.

Scheduled on separate day from planned procedures per insurance rules.

May be added to a procedure same day if extra care is required in light of the procedure or an acute problem arises on the same day (ex. anaphylaxis care post procedure).

Some capitated plans (ex. Aetna, United Healthcare) do not honor more than 25-29 minutes of medical service per day per patient, so comprehensive care (ie. yearly check-ins) must be divided into multiple visits if you would like us to bill your insurance.

Patient payments and payment plan options



Learning about your insurance plan and who pays for what, including deductible costs and coinsurance costs, is your responsibility but we are certainly willing to help guide you.

Our staff will be asking for updated insurance cards and copays at every check-in.

Upon check-in, we will also ask for a security deposit or up to 1/2 of your estimated cost if your deductible has not been met (whichever is greater), or for your projected co-insurance amount. If over $300, we will collect the $300 and start a payment plan for the remainder.

Email links to our statement and payment portal will be sent monthly. Paper statements are available at check-in and upon request.
